"It will never happen to me" is often stated by risk managers, engineering managers and safety professionals or anyone responsible for facilities with fuel-fired equipment that live within a culture of lack of knowledge, misunderstandings and/or denial about the impact of an explosion or fire caused by the operation of equipment such as boilers, furnaces, ovens, dryers, etc.
Boiler Explosion
Hundreds of explosions happen every year resulting in millions of dollars in business interruption, facility damage, lawsuits, fines, litigation and lost market share. Conversely, smaller, but more frequent, production outages also cost millions in business interruption, supply chain delays, lost orders and competitiveness, but are often deemed to be culturally accepted as a general business practice.

Matrix Risk Consultants has found that some companies have combustion equipment safety programs that go well beyond the minimum, legally mandated requirements. These safety programs include a detailed check of the combustion systems, checks which include an analysis for code compliance, installation deficiencies, interlock testing, screening for maintenance practices and assessing technological advances. Many facilities, however, do not have personnel properly trained in combustion equipment maintenance, start-up or shutdown procedures and/or equipment operations and do not follow proper interlock and safety testing guidelines even though they are mandated by law.
